Volunteers 15 years of age and under must be accompanied by an adult at all times.
Volunteers 16-17 years of age must have a parent or guardian sign this form: City of San Jose Volunteer Agreement Form for Minors.  (105 KB, Adobe .pdf file). When complete, submit to volunteer@grpg.org or fax to 866-418-8765.
